The 36-year-old, who is Chelsea's all-time leading goalscorer with 211, earned 106 England caps and has been honoured for services to football.
He spent last season at Manchester City but leaving to play Major League Soccer for New York City this summer.
"I am delighted to receive this huge honour," said Lampard, who won 106 England caps. "I feel very fortunate to have had such a long and fulfilling career in football.
"This is an extremely proud moment for myself and my family."
